1002-1(a)(2):   Mailing Address: Filing an Update 

LBR 1002-1(a) provides a procedure for a debtor or other party to file a form titled "Change of Address" to indicate that the party has a new address for receiving regular mail.

The court provides a mandatory form.  See link below.

The form must be used by debtors and creditors.
