LBR 3017-1 provides requirements in a chapter 11 case when a debtor or trustee files a motion to approve the adequacy of a disclosure statement.

  • The court form linked below is optional when a chapter 11 bankruptcy case has been filed.
  • This form can be used by debtors who are a corporation, a partnership, or other entity.
  • Some judges require use of their forms in disclosure statement proceedings.  Check your judge's webpage.
