LBR 1017-1(a)(3) provides the procedure for a chapter 11 debtor to request an order converting a chapter 11 case to another chapter

  1. A fee may be required.  Please consult the fee schedule.  See link below
  2. A motion is required .  The court provides a form motion -- See link below 
  3. A hearing is not required
  • The parties must file a Notice of Motion for Order Without Hearing under LBR 9013-1(p)
  • The court provides a form notice -- See link below 

Before a chapter 11 case is converted, all pending court costs and U.S. trustee fees must be paid.   When a motion to convert a chapter 11 case is filed, the court may file a Notice of Court Costs if court costs remain unpaid and the U.S. trustee may file a notice to indicate the amount of U.S. trustee fees that remain unpaid.   

FRBP 1019 provides instructions on the documents that must be filed after a chapter 11 case is converted or reconverted to a chapter 7 case.
